To begin, patients must schedule an appointment with a designated civil surgeon at Family Health Center of Plainfield & Med Spa. During the visit, a full medical history is reviewed, followed by a physical exam, tuberculosis screening, and any required blood work. The provider will assess for communicable diseases, mental health status, and vaccination history. If vaccines are missing, appropriate immunizations are administered. The completed I-693 form is sealed for submission to USCIS. All evaluations follow federal guidelines and are completed promptly to help applicants stay on track with immigration deadlines.
